CSI Flexible Benefits Plan Administration Manual
Enclosed is the new administration manual for the CSI Flexible Benefits Plan. We encourage you to spend some time reviewing the manual to become familiar with the type of information it contains.
Request for Reimbursement Forms and Change in Benefit Election Forms
In the front pocket of the Administration Manual are extra Request for Reimbursement Forms and Change in Benefit Election Forms. Additional forms can be printed from the web site.
Also included are instructions that you and your participants can use to access the web site. You can use the web site to review account balances and print additional forms. If you are the school’s contact person and are not a participant in the Plan but would like access to the web site, contact me and provide your social security number. You will be set up with administrator access.
Summary Plan Description
The Summary Plan Description (SPD) is located under Section 7 of the Administration Manual. Federal law requires that each participant receive a copy of the SPD.
Changes in Benefit Elections
Changes in benefit elections may be made during the Plan Year only if there is a change in status as described in Section 1, page 6 of the Administration Manual. There is additional information on the Change in Status Matrix located in Section 3, Exhibit N. It is important to note that if a participant has a change in status and wishes to change the current election, the Change in Benefit Election Form must be completed within 30 days of the change.
