Deadline for Incurring and Submitting Claims
The deadline for incurring claims in the current Rainbow Plan Year is August 31, 1997. To avoid forfeiting the funds you have contributed, now is a good time to review your account balance and the claims you have incurred or expect to incur prior to August 31, 1997. You may double check your account balances by contacting our Plan administrator, HRM Claim Management, Inc., at these numbers:

To avoid forfeiting funds, submit claims for reimbursement as they are incurred. Claims may be submitted for the current Plan Year until November 26, 1997. However, claims must have been incurred by August 31, 1997.
New Request for Reimbursement Forms
The Request For Reimbursement form has been revised so that claims may be processed more quickly.
Your school has been provided with a supply of revised forms. When submitting a claim, please use the Request for Reimbursement form which has a revision date of 3/97 in the lower left-hand corner. Discard any old forms that you may have.
Change in Eligibility for Dependent Day Care Expenses
There has been a change in eligibility for kindergarten expenses for the 1997/1998 Plan Year. Kindergarten expenses no longer qualify as eligible expenses and cannot be reimbursed under the Rainbow Plan. Kindergarten expenses will continue to be reimbursed for the current Plan Year.
