Rainbow Plan Enrollment Deadline

In order for enrollment forms to be processed by the new Plan Year the enrollment package must be received in our office by August 9, 1996.  If it is not possible to complete the enrollment package by August 9, please contact our office.  If an employee needs to make a change after August 9, a revised enrollment form may be sent or faxed any time prior to August 31.


Be sure your enrollment package includes these items: 

  • Information Sheet
  • Authorization Letter
  • Key Employee List (must be signed even if there are no key employees)
  • Enrollment Form signed by each employee authorizing or waiving participation


New employees have 30 days from their hire date to enroll in the Rainbow Plan.  Please note that the enrollment form must be signed on or prior to the first of the month in order for the employee to participate in the Plan for that month.



Changes in Benefit Elections during the Plan Year 

Under federal regulations benefit elections may not be changed during the Plan Year unless there is a change in family status.  Changes in family status are described in the Rainbow Plan Administration Manual on pages I-4, II-4 and II-5.  If an employee experiences a change in family status he or she needs to complete a Change in Benefit Election form within 30 days of the change and forward it to our office.  The form must be signed on or prior to the first of the month in order for the change to be effective that month.
